New Proposed Structure File Number (SFN) Request

Please fill out the following information in order to request a new Structure File Number (SFN). All fields marked with * and yellow background must be filled out. Fill out other fields as needed.

Refer to the Ohio Bridge Inventory Coding Guide for details on when to code a new SFN.

Fill out a form for each SFN for each status change.  

Response-time from the AssetWise.Support@dot.ohio.gov will typically be within 2 business days and not to exceed 10 business days.

The four stages of an SFN are below:

  1. Proposed - These are new SFN's typically requested in the design phase and the bridge does not yet exist.  The Asset Name will have the word "Proposed" as a suffix so users can quickly find them in AssetWise.  All updates to the inventory must be made in the inventory in the "Edit Asset Values" button on the Asset Details page and NOT in the "Create Report" button inspection report.  The inventory shall be completed before the bridge status will be changed to "Active."  Some of these fields must be completed by AssetWise.Support@dot.ohio.gov therefore users MUST coordinate prior to making active by completing the appropriate form.
  2. Inactive - These are bridges that are partially constructed and have a full or almost-complete inventory and not-yet-open to traffic.  It is not uncommon for an SFN to skip this phase.  The Asset Name will have the word "Inactive" as a suffix so that users can quickly identify the SFN in AssetWise. All updates must be made in the inventory in the "Edit Asset Values" button on the Asset Details page and NOT in the "Create Report" button inspection report.   The inventory shall be completed before the bridge status is changed to "Active."  Some of these fields must be completed by AssetWise.Support@dot.ohio.gov therefore users MUST coordinate prior to making active by completing the appropriate form.
  3. Active - SFN is an in-service bridge carrying active traffic.    The first inspection report is created after the SFN is made active.
  4. Retired - SFN is no longer a bridge carrying public traffic or is out-of-service or removed entirely.  The public entity may require out-of-service bridges to remain in the active status based on maintenance agreements or funding delays.  In such case the structure still requires regular inspections as if it were a bridge.
