Make New SFN Requester Information

Please read the steps below as some instructions have changed.  Once a unique Structure File Number (SFN) is created then that series of numbers is locked.  

I. Make-New Request:  Fill out this form request a unique SFN.  This SFN will be placed in the Proposed or Inactive status.  Expect 2-10 business-day response-time.  Do not email Kammy directly for Make-New requests.  Fill out the form below to ensure a successful make-new SFN transaction. 

II. Work to be done After a Make-New Request:  After the Bridge is constructed, then the entire inventory and the initial inspection must be performed.  Data must be entered within the "Edit Asset Values" button in AssetWise.  A separate Make-Active form must be filled out to request the SFN to be made Active.  After the SFN has the initial inspection and is made-active then the "Create Report" button may be used. Until then do not create reports.

Bridges that must be included in the Federal Submission include: ACTIVE STATUS, OPEN TO PUBLIC VEHICULAR TRAFFIC, CLEAR SPAN MORE THAN 20-ft. All others will not be included in the Federal Submission. (259) SFN Creation Date(s) & (260) SFN Retire Date will be visible in the bridge record on the Ohio Bridge Inventory form.
SFN's in "Proposed" and "Inactive" status will be identified by the word Proposed or Inactive in the Asset Name so you can find and identify them quickly in AssetWise. Retired bridges are available in the Collector>Archived Assets in AssetWise.
