Please read the steps below as some instructions have changed. Once a Structure File Number (SFN) is made Active it is immediately subject to State and Federal Quality Control Metrics. Following the steps below will ensure a successful and compliant 'Make-Active' transaction.

I. Work to be done before a Make-Active request:

If the SFN you want to make-active does not yet exist then STOP and first fill out the "Make New SFN" form to create a new SFN with "Proposed" status.
The SFN must already exist in the "Proposed" status. A "Proposed" SFN is typically in the design phase or construction and is near completion.
Complete the bridge inventory data on the SNBI form (SNBI sections 1-5) within the Edit asset values button on the Asset Details page . Initial inventory MUST be done through EDIT ASSET VALUES. If you create an inspection report before the SFN is made active, then your work in the in-progress report will not carry-over into the inventory and the report will be deleted.
Once SNBI bridge inventory is complete, you can submit a "Make Active SFN Request."
- Inspection data (condition ratings, inspection dates, element level data) will be filled out in an inspection report during the initial inspection.
The updated Ohio Bridge Inspection Manual chapter 7 details the full required bridge inventory.

II. Make-Active Request: Fill out this form to request the Proposed SFN be made active. Expect 2-10 days response time. Please do not email Matt Stefanik directly for make active requests. Fill out the form below.

III. Work to be done Immediately After a Make-Active Request: Once the bridge is made active, an Initial inspection MUST take place within three months of the bridge opening to traffic. The bridge does not need to be open to traffic to perform an initial inspection. This is the first inspection date for the bridge and SNBI Initial Inspection type MUST be chosen. During the initial inspection, baseline conditions for condition ratings and element level ratings (if applicable) will be recorded.
