Make Active SFN Requester Information

Please read the steps below as some instructions have changed.  Once a Structure File Number (SFN) is made Active it is immediately subject to State and Federal Quality Control Metrics.  Following the steps below will ensure a successful and compliant 'Make-Active' transaction.  

I. Work to be done before a Make-Active request: 

  1. If the SFN you want to make-active does not yet exist then STOP and first fill out the "Make New SFN" form to create a new SFN with "Proposed" or "Inactive" status.  
  2. The SFN must already exist in the "Proposed" or "Inactive" status. Either status is sufficient for a Make-Active request.  A "Proposed" SFN is typically in the design phase and the bridge doesn't yet exist.  An "Inactive" bridge exists or construction is near completion.  
  3. Physically go to the bridge and perform the Initial-Inspection in the field while the SFN is in the "Proposed" or "Inactive" status to assess it's component/element condition and have the first inspection dates to enter.  You will not be able to use the IPAD version to enter the inspection information (yet) and do not select the button "Create Report".  The first inspection must be performed immediately prior or within 90-days of the Date-Open-to-Traffic, including the first phase in a part-width construction.  
  4. Update the inspection and inventory data in the Edit asset values button on the Asset Details page .  This includes the inspection date(s), the condition ratings and the element ratings (if element data is required).  If you create a report before the SFN is made active then your work in the in-progress report will not carry-over into the inventory and Kammy will need to delete your report.  
  5. Calculate your appraisal fields and the sufficiency rating by selecting the "NBI Calculations" form.  Check your inventory for obvious errors by selecting the "NBI Error Check" form. Expect errors on blank fields that will be updated by Kammy when the inventory is reviewed.  These fields include: State Code, District, County and Structure File Number. 

            

           

II. Make-Active Request:  Fill out this form to request the Proposed or Inactive SFN be made active.  Expect 2-10 business-day response-time.  Do not email Kammy directly for Make-Active requests.  Fill out the form below. 

III. Work to be done Immediately After a Make-Active Request:  After the SFN is made Active THEN create an inspection report using the create report button on the Asset Details page , reenter the routine inspection date and submit the report for PE review.
